In November 2018, Green publicly revealed that he had been diagnosed with ALS (amyotrophic lateral sclerosis), a progressive neurodegenerative disease, two years earlier. He has stated that he believes the condition may be linked to his football career, although the medical community continues to study such connections. Despite his diagnosis, Green remains active in advocacy and public service. He founded the Tackle ALS fundraising campaign, which has helped raise more than $10 million for ALS research. Although ALS has taken his speaking voice, he continues to write books using eye-tracking technology, and since 2024 he has co-hosted the podcast Nothing Left Unsaid with his son Troy, using an AI recreation of his original voice. He currently serves on the board of the Golisano Children's Hospital in Syracuse, New York, and also contributes to the National Writing Project, supporting writing and literacy development nationwide.
